It is essential to register a child who was born outside Sri Lanka, at the Embassy of Sri Lanka if the child’s name is to be entered in the passport of one of the parents.

 

Registration of births should be made within 3 months of the birth. Where an application for registration of a birth is made after 3 months and within one year from the date of birth, the reasons for the delay in making the application should be submitted to the Embassy.


The birth should be registered within a period of one year from the date of birth of the child. In the event of failure to do so, a fine shall be imposed for the delay.

 

 

For the registration of birth, the following documents should be submitted:

 

(1) Duly filled Application Form B4, if the application is made within 03 months of birth - Download Application Form

     Duly filled Application Form B6, if application is made after 03 months of birth

(2) Duly filled application for registration of birth outside Sri Lanka – Form: Citizenship 1

The forms mentioned in (1) and (2) above should be completed in duplicate.


(3) Declaration of citizenship by father / mother – Declaration Form  

This form should be filled in triplicate.

 

 

(4) Birth certificate of the child (original) issued by the competent local authority in the country of birth of the child

 

(5) Birth certificates and passports of the parents

 

(6) Marriage certificate of the parents



Fees payable for registration of birth are as follows:

 

For registration of birth - RMB 60.00

 

For issue of birth certificate or each additional copy there of - RMB 105.00

 

For delayed birth registration (per year) - RMB 30.00
